2015-2016 Kindergarten Immunization Requirements
To attend kindergarten, a student must have written proof of receiving the following immunizations:
- 5 DTaP/DT
- 2 Hepatitis A
- 3 Hepatitis B
- 4 Polio
- 2 (two) Varicella (Chickenpox)
- 2 MMR (Measles, Mumps, Rubella)
The 5th DTP and 4th Polio immunizations need to be given after the child’s 4th birthday; the 1st MMR shot must be given after the child’s first birthday. Parents may sign the appropriate box on the pink immunization card indicating their child has had the Chickenpox disease instead of the shot.
Kindergarten Registration for Spanish Oaks Elementary will be held on Tuesday, March 17, 2015 from 7:30 a.m.-4:00 p.m. Please bring proof of current immunizations, even if all are not complete.
